The process in which there was most likely a malfunction is GAMETOGENESIS.
Gametogenesis is the process by which the female and male sex cells are divided during the process of meiosis in order to form matured haploid gametes. Gametogenesis occurs in stages and any error in the division of the germ cells during the meotic divison will result in errors in the number of chromosomes that will be formed.
